Notification
In the event of a work-related injury, the employee should comply with the following procedures:

Employee and/or co-worker should evaluate the nature and seriousness of the injury. If life-threatening, immediately dial 911.

Injured employee, if capable, or a co-worker must immediately notify his or her supervisor.
Important: Prompt reporting of the injury to the supervisor, State of Arizona's early claims reporting service, and the NAU Human Resources office will accelerate the processing of your claim and avoid unnecessary delays or denials of possible benefits. A claim must be filed within one year of the date of injury.

An employee who works on campus may visit NAU's convenient Campus Health Services clinic if medical care is necessary or may choose a private physician to treat his or her industrial injury (Workers Compensation Physician Information Sheet). In emergency situations, the employee should go to a private physician, the Flagstaff Medical Center emergency room or similar facility. An employee who works at an NAU location outside of the Flagstaff area should go to a private physician, closest emergency room, or similar facility.

A Supervisor's Report of Industrial Injury (SRI) form must be completed by the supervisor and employee (if able) the same day as the accident. This provides the initial paperwork necessary to review claims for worker's compensation benefits, and allows review by NAU's Loss Prevention Committee of potentially unsafe environments or circumstances that might be prevented in the future. All injuries must be documented whether they are minor (only requiring minimum first aid treatment, or no treatment), or require further medical treatment and/or involve lost time.

If accident resulted in exposure to bodily fluids of another person or other potentially infectious biological material, the employee (if able) must complete and submit this signed Report of Exposure form to his/her supervisor or human resources within 10 days of the exposure.

Supervisors are required to utilize the State of Arizona's Early Claims Reporting Service phone line to report all work-related injuries/illnesses prior to the end of the shift they occur. The toll free number is 1-800-837-8583 and is available seven days a week, 24 hours a day. When a call is placed to the Early Claims Reporting Service phone line, you will be prompted to answer several questions.
- In addition to calling the Early Claims Reporting Service phone line, the supervisor is required to submit the original copy of the Supervisor's Report of Injury (SRI) to the NAU Human Resources office the same day the injury occurs.
