A Qualified Life Event is an event that gives an employee the opportunity for employees to make a change(s) to their benefit plan. The type of life event the employee experiences will dictate the change(s) available to the employee. Examples of Qualified Life Events include the following:
Change in Marital Status
- Marriage
- Divorce, legal separation, annulment
Change in Dependent(s)
- Birth, adoption or legal guardianship
- Change in dependent eligibility
- Over age 19 or 25
- Change in full-time student status (age 19-25)
- Dependent's marriage
- Loss/gain coverage - Spouse
- Loss/gain coverage - Child(ren)
- Removal of legal custody - dependent
- Death of spouse/dependent
31-Day Deadline
Employees have 31 days from the date of the Qualified Life Event to notify the Human Resources Department, make changes to their benefit plan in the LOUIE system, and submit all supporting documentation. Failure to meet the 31-day deadline may create the following results:
- Jeopardize your eligibility to make changes to your benefit plan;
- Loss of coverage for you and/or your eligible dependent(s);
- Loss of COBRA rights for your dependent(s); or
- Other negative impacts to you and/or your dependent(s)
Pursuant to the Children's Health Insurance Program Reauthorization Act (SCHIP), the following events are considered qualifying life events as of April 1, 2009. Under provisions of the Act, employees have 60 days following the event to request enrollment in the Benefit Options plan.
- employee or dependent loses Medicaid/SCHIP coverage.
- employee or dependent becomes eligible for Medicaid/SCHIP premium assistance.
Notify Human Resources
Employees notify the Human Resources Department by submitting an on-line Qualified Life Event Form. Once the Human Resources Department receives your on-line form, we review it and open your access to make changes to your benefit plan.*
In addition to submitting the Qualified Life Event Form and making changes in the LOUIE system, certain supporting documentation may be required. Please refer to the Qualified Life Event Form for guidance regarding required documentation. The Human Resources Department MUST receive all supporting documentation to complete the process.
Making Changes
Once the Human Resources Department receives the on-line Qualified Life Event Form; the employee will be able to make changes to their benefit plan directly through the LOUIE system*. Log in to the LOUIE system at https://peoplesoft.nau.edu and follow the path below:
Self Service / Benefits / Benefits Enrollment
On the Benefits Enrollment page there will be an "Event" listed. It will indicate "Qualified Life Event", and there will be an Info and Select button available. The Select button indicates access has been granted. Click the Select button to make changes in your benefit plan. If an event is not listed or the event listed is not "open", please contact the Human Resources Department.
The LOUIE system is programmed to allow only those changes to the benefit plan the Qualified Life Event dictates**. If you have questions about the changes available to you or if you are having difficulty navigating the system, please contact the Human Resources Department at (928) 523-2223.
*Note: The process for setting up employee access to changing their benefit plan in the LOUIE system may take up to 3 working days. If it has been over 3 working days since you submitted your notification and you are unable to make changes, please contact the Human Resources Department.
**Note: Specific instructions related to each Qualified Life Event will be sent directly to you once the QLE form has been received by Human Resources.
