Notifying Human Resources Employees notify the Human Resources Department by submitting an on-line Qualified Life Event Form. Once the Human Resources Department receives your on-line form, we review it and open your access to make changes to your benefit plan.*
In addition to submitting the Qualified Life Event Form and making changes in the LOUIE system, certain supporting documentation may be required. Please refer to the Qualified Life Event Form for guidance regarding required documentation. The Human Resources Department MUST receive all supporting documentation to complete the process. |
