Ensure your Eligible Dependents Stay on Your Medical Plans   Print 
   
 
Every academic year employees who have a child between the ages of 19-25 on your medical, dental, and/or vision plan(s) must submit proof of full-time student status to Human Resources.  Providing full-time student status documentation now will ensure continuous coverage for your child.
 
Employees are responsible for notifying the Benefits staff of any change in their dependent child’s eligibility.  Failing to provide student status to Human Resources will result in a loss of coverage for your dependent between the ages of 19-25.
 
Here are the steps to efficiently ensure there is no lapse in coverage for your child:
  • As soon as your child has registered for the Fall 2008 semester, have your child obtain a copy of the student schedule or a letter from the school’s registrar’s office verifying full-time enrollment (for state universities, 12 credit hours is considered full-time). Students can also print their school schedules directly from PeopleSoft at SA Self Service >> Learner Services >> Academics >> View My Class Schedule.
  • Mail a copy of the full-time status to NAU PO Box 4113, Flagstaff, AZ, 86011, Attention: Benefits; or fax a copy of the documentation to (928)523-8861, Attention: Benefits.  BE SURE TO INCLUDE THE EMPLOYEE’S NAME, EMPLOYEE ID, AND THE DATE OF BIRTH OF THE DEPENDENT ON THE DOCUMENT.  This will allow Human Resources to forward the proof of full-time student status to all of the applicable carriers.
  • If your child is NOT going to be a full-time student for the Fall 2008 semester, please complete a “Qualified Life Event” request form at http://hr.nau.edu/apps/life_event/form.php .  This will allow us to offer your dependent child continuous coverage through COBRA. 
Employees are responsible for notifying the Benefits staff within 31 days of a change in eligibility. At that time, COBRA will be offered to the dependent and the employee/dependent has 60 days from the date of the child’s loss of insurance coverage to choose to continue one or more of their insurance plans. The coverage can be extended up to 36 months by paying the COBRA premium.  If the Benefits Department is not notified of the change in eligibility within the 31-day window, the COBRA rights are forfeited and there is no opportunity to extend coverage through the NAU plans.
 
Domestic Dependents
 

The state of Arizona is extending health benefits (medical, dental, and vision) to the following qualified dependents who meet certain criteria established by the Arizona Department of Administration (ADOA):

  • domestic partners,
  • children of domestic partners,
  • older children (19-24) who are not full time students.

IMPORTANT!
Employees who plan to enroll any of the above dependents should not proceed with the online benefits enrollment. A paper enrollment is required for both the employee and all dependents of that employee, along with supporting documentation.

Frequently Asked Questions:
 
1) What is the deadline for the employee to provide the documentation?
Employees have 31 days after the child’s birthday to provide documentation of full-time student status to Human Resources. However, if the documentation is turned into our office as soon as the dependent is registered for the Fall 2008  semester, there will be no interruption in your child’s insurance coverage.
 
2) What if my child is not registered as a full time student for the Fall 2008 semester?
A dependent child who is 19 or older is only eligible to remain on your insurance plans if he/she is a full time student.
 
3) What other benefit rules apply to children of NAU employees who are over 19 years old?
A child is not eligible for coverage under your NAU insurance once they turn 25 years of age, regardless of their student status.
 
A child is not eligible for coverage under your NAU insurance once they turn 25 years of age.  
 
If your dependent child is disabled, special rules apply and you should contact the Human Resource department.
 
A child who is married is not eligible to remain on your NAU insurance plans.
