Information Change - Dependent/Beneficiary   Print 

Employees can update their dependent or beneficiary information directly through the LOUIE system. Log into the LOUIE system at https://peoplesoft.nau.edu and follow the path below:

  • Home / HR Self Service / eBenefits / Dependents / Dependent Beneficiary Summary

On this page an employee will be able to view a summary of dependent/beneficiary information. Click on the name to view Personal Information. Click Edit to update the information.

Name Change - Dependent

Employees who have a dependent(s) with a name change must first apply for a new Social Security Card with the Social Security Administration (SSA). SSA can be reached by calling 800-772-1213 or visit the local Flagstaff office located at 1585 S. Plaza Way, Suite 130.

To update a dependent's benefit information employees must submit an on-line Qualified Life Event Form. Complete and submit it to the Human Resources Department. A copy of the new Social Security Card will be required in order to process the request.

Name Change - Beneficiary

Employees who have a beneficiary(s) with a name change can update their benefit information by submitting an on-line Qualified Life Event Form. Complete and submit it to the Human Resources Department. Additional documentation is not required.
