Avesis Vision and the Blue Cross Blue Shield (BCBS) PPO Vision Benefit

Many employees find themselves confused by the Avesis Vision plan and the vision benefit available through NAU’s BCBS PPO plan. This is understandable because both the vision plan and the BCBS vision benefit are provided through Avesis Vision Inc. Although they are provided by the same vendor, the Avesis Vision plan and the BCBS vision benefit are separate, which means they can be utilized individually or together. We have found that some employees choose to elect the Avesis Vision Plan, utilize the benefit to the maximum and then submit any out-of-pocket costs to BCBS for reimbursement. Other employees choose to waive the Avesis Vision Plan and use the BCBS benefit for all their vision needs. Below is a brief overview of the two benefit plans.

Avesis Vision

The Avesis Advantage Vision Plan is part of the Saguaro Benefit Plan available to all state employees. This plan provides vision coverage including an examination and a contact or frame benefit. Employees pay a monthly premium of $6.48 (single) or $17.54 (family) for this benefit. BCBS Vision Benefit

The vision benefit available through NAU’s BCBS PPO plan is included in the medical plan. There is no additional premium cost to the employee. The benefit is a basic cash reimbursement benefit. BCBS reimburses 50% of out-of-pocket expenses for routine eye exams, glasses or contacts up to a maximum reimbursement of $250 every 24 months. The 24-month period begins on the first date of service.

If you do not know if you elected the Avesis Vision Plan, you can view your benefit elections thought the LOUIE system. Simply log into LOUIE at https://peoplesoft.nau.edu and click through the following links: HR Self Service > eBenefits > Insurances > Insurance Summary.

Note: Unlike NAU’s BCBS helath plan, the CIGNA plans available to State employees do not include a vision benefit.
