ARS 23-908 (F) requires NAU to inform State Risk Management and the Industrial Commission of the injury within ten days after notification. To initiate a claim, both an Employer's Report of Injury (101) and a Physician's Report of Injury (102) MUST be submitted. The Physician's Report of Injury is processed by the treating physician. Copies of both forms are provided to the Industrial Commission, Employer, and State Risk Management. When the claim is complete and filed, medical treatment, testing and compensability can be determined.
Once the Industrial Commission receives the Physician's Report of Injury (102), State Risk Management has 21 days to accept or deny the claim. If the claim is accepted, the injured worker may be eligible for one or more of five types of benefits.
