In the event of a work-related injury, the employee should comply with the following procedures:
1. An assessment of the injury should be made by the employee or co-worker.
2. Injured employee, if capable, or a co-worker must immediately notify his or her supervisor.
3. An employee who works on campus may visit the Fronske Health Center if medical care is necessary or may choose a private physician to treat his or her industrial injury (Workers Compensation Physician Information Sheet). In emergency situations, the employee should go to a private physician, the Flagstaff Medical Center emergency room or similar facility. An employee who works at an NAU location outside of the Flagstaff area should go to a private physician, closest emergency room, or similar facility.
4. A Supervisor's Report of Industrial Injury (SRI) form must be completed by the supervisor and employee (if able) the same day as the accident. The SRI is a form which provides the initial paperwork necessary to provide worker's compensation benefits, allows review by loss control of unsafe acts that might be prevented in the future, and provides information necessary to investigate questionable claims. An injury, whether it be minor and only requiring first aid treatment, or one that requires further medical treatment and/or involves lost time, must be documented.
5. Supervisors are required to utilize the State of Arizona's Early Claims Reporting Service phone line to report all work-related injuries/illnesses prior to the end of the shift they occur. The toll free number is 1-800-837-8583 and is available seven days a week, 24 hours a day. When a call is placed to the Early Claims Reporting Service phone line, you will be prompted to answer several questions.
6. In addition to calling the Early Claims Reporting Service phone line, the supervisor is required to submit the original copy of the Supervisor's Report of Injury (SRI) to the NAU Human Resources office the same day the injury occurs.
Important: Prompt reporting of the injury to the supervisor, State of Arizona's early claims reporting service, and the NAU Human Resources office will accelerate the processing of your claim and avoid unnecessary delays or denials of possible benefits. A claim must be filed within one year of the date of injury.
