Individual Life
Supplemental individual life insurance may be purchased in increments of $5000.  The maximum amount available for purchase is three times your annual salary, or $300,000, whichever is less.    

New Hire Enrollment
New employees may elect up to the maximum amount at their initial enrollment. 

Annual Changes at Open Enrollment
During open enrollment, employees may elect coverage, increase/decrease coverage or stop coverage.  Elections or changes in coverage may be made in multiples of $5,000.  The maximum annual election or increase available is $20,000.  

Qualified Life Event Changes
Certain qualifying life events will allow employees to newly elect or change their individual supplemental life insurance.  The same election or change rules apply as stated above.

Note: Standard supplemental life is in addition to the BASIC Life policy provided to all benefit eligible employees.  Please do not include the BASIC Life coverage when calculating your supplemental coverage.


Dependent Life
Life insurance coverage is available for your spouse and eligible dependent children. Coverage may be elected at: $2,000. $4,000, $6,000, $12,000, or $15,000. You do not have to purchase Standard supplemental life for yourself in order to purchase Standard dependant life insurance.  If you decide to elect coverage for your dependents, you are automatically the beneficiary.  In the event of your death, benefits are paid to your designated beneficiary(ies).

Premium Rates
The premiums for individual supplemental life insurance are paid on a before and after-tax basis.  Coverage above $35,000 is paid on an after-tax basis.  Rates are based on age brackets and may change annually.  Please check your rate each year.

The premiums for dependent life are paid on an after-tax basis.  The premium you pay covers all your eligible dependents, no matter how many eligible dependents you have.
